FAQ about hysterectomy recovery rehabilitation with a physiotherapist

Our women’s health physio specialist answers all you common questions about post hysterectomy recovery, if you have further questions, she will be happy to help you understand everything you need about your hysterectomy recovery during your initial assessment and appointment

When can I start my hysterectomy recovery rehabilitation after my surgery?

You need to wait between 6 to 8 weeks before starting your rehabilitation. Your body, first, needs to recover from the surgery so it is important to take a good rest before anything. Then around 6 weeks post hysterectomy your scar will have heal and our physiotherapist will be able to make a thorough assessment and a personalized treatment plan based on your history and situation when she will see you and to start giving you exercises and her hands-on treatment.

What should I expect after a vaginal hysterectomy or vaginal surgery?

Immediately after your surgery, you might have a catheter, a small tube placed in the bladder to drain your urine into a bag. You might also have a gauze inside the vagina to prevent bleeding. Your doctor will tell you how long these should stay in place, usually between 8 and 24 hours.

Will I feel Pain after a hysterectomy surgery and can physiotherapy help with the pain ?

You might feel some pain or discomfort in the groin, the vagina and the lower abdomen after the surgery. If you had a sacrospinous suspension (for a uterine or vaginal vault prolapse) you might also feel sharp or persistent pain deep in the buttocks. This pain or discomfort usually goes away within one or two weeks but it can last several weeks that will be treated with pain medication. If you still feel some areas being painful when you start your physio rehab, our women’s health physio will adapt her exercises plan and treatment to first relieve you from the pain before starting more advance treatment.

Is it normal to have leakage or constipation after a hysterectomy? Bladder and bowel function after surgery

Leakage and incontinence are not uncommon, mainly at the early stage post surgery but it should resume as normal once your body recover. If you experience leakage or incontinence after 2 to 3 weeks, a few sessions of pelvic floor rehabilitation will help you stop the leakage.

After a vaginal surgery, you might also notice that your urine flow is slower and that your bladder empties more slowly than usual. About 5 to 10% of women have difficulty emptying their bladder completely after surgery. You

Also, constipation is common after surgery. Before and after the operation, eat plenty of fruit, fibre and drink enough liquids to soften your stools. Stool softeners (laxatives) are often prescribed after surgery to prevent constipation. Take them regularly at the beginning of your recovery at home. It is important not to push too hard when having a bowel movement as this could put pressure on your stitches inside the vagina. Some women feel a burning or sharp pain in the rectum after the surgery. This usually goes away within a few days.

Should I expect vaginal discharge after the surgery?

Yes, it is normal to have some bleeding followed by white discharge for up to six weeks after the surgery as your vaginal stitches dissolve.
At first, the bleeding might be bright red, then it usually turns to a darker brownish red. The amount can vary from day to day. If you have heavy bright red bleeding or clots that make you change your pad frequently, contact your doctor. Use sanitary pads and not tampons during the first six months after the surgery. So to resume, it is normal but must be followed.

When can I start my favorite sport or activities after the surgery

After the surgery, you will feel more tired than usual so make sure to rest as much as possible and listen to your body. Start by walking at home, then when you feel ready, increase your activity and try to go for daily walks. Walking is a good physical activity because it puts little pressure on the scar.
Avoid Fitness exercise such as running, fast walking, aerobics, etc. for at least six weeks after surgery. Ideally, wait until you have done you pelvic assessment with a physio before start sports so you are sure that you pelvic floor and core are strong enough to support your activity and not increase your pain or incontinence. You can start or restart pelvic floor exercises when you feel able to, usually one to two weeks after surgery.
